Health Procedures for Registration of NEW or TRANSFER Students

A copy of the student’s up-to date immunization record must be on file in the school health office before the student is allowed to attend any classes.
  1. Parent/guardian will receive forms to complete.
    • Student Health History Form.
    • Emergency Dismissal Form- Forms signed by the parent or guardian MUST be on file in the School Nurse’s Office. The information requested is critical at time of illness or emergency. Parents are required to update this information throughout the school year as information changes.
  2. New admissions to Holliston High School (all grades) must have documentation of a valid physical examination within a year prior to entry date and must be accompanied with an up-to-date immunization card.
  3. It is the responsibility of the parent/guardian to notify the school nurse in writing of any health condition pertinent to the child’s well-being, such as tendency to nosebleed, seizure disorders, allergies, reactions to bee stings, diabetes, joint and bone disorders, difficulties with speech, vision or hearing and religious stipulations regarding medical care.

All medications that must be taken by students during the school day must be administered by the School Nurse or designee in accordance with Holliston Public Schools policy.
